These terms cover using My Care Hub. We've written them to be understandable, because terms nobody can read protect nobody.

My Care Hub is not medical care
The app helps you record and share information. It does not diagnose, treat, or give medical advice, and it is not a substitute for a clinician. In an emergency, contact emergency services — not the app.

This is important. My Care Hub helps you record and share information. It does not diagnose, treat, or give medical advice, and it is not a substitute for a clinician.
In an emergency, contact emergency services. Do not rely on the app, a QR code, or the AI assistant in an emergency.
Providers reading a shared profile should treat it as helpful context from the patient — not as a verified clinical record.
Your account
Keep your login details to yourself, and tell us if you think someone else has access. You're responsible for activity on your account.
Your content is yours
Everything you put in a profile stays yours. You give us only the permission we need to store it, display it to you, and share it when you ask us to. That permission ends when you delete the content.
Managing someone else's profile
If you create or manage a profile for another person, you confirm you have the authority to do so, and you agree to keep their voice central and respect their preferences about their own information.
Accuracy
You control what's in your profile, so you're responsible for keeping it accurate and current. Out-of-date medication or allergy information can be worse than none.
Providers, events and groups
Listings are provided by the organizations themselves. We don't endorse, verify or guarantee any provider, and we're not responsible for the care they give or the accuracy of what they publish — including their stated accessibility features. Use your own judgment.
The AI assistant
The AI assistant is coming soon. Once available, it will explain terminology in plain language. It can be wrong. Don't use it to make medical decisions, and check anything that matters with a clinician.
Acceptable use
Don't use My Care Hub to break the law, to impersonate someone, to access a profile you weren't given, to scrape or resell data, or to interfere with the service. Don't upload other people's information without the right to do so.
Availability
We'll try to keep the app running, but we can't promise it will never be down. Keep a copy of anything you can't afford to lose — you can download your QR code and export your data.
Liability
To the extent the law allows, we're not liable for indirect or consequential loss arising from your use of the app. Nothing here limits liability that cannot legally be limited — including for death or personal injury caused by negligence.
Ending it
You can stop using My Care Hub and delete your account whenever you like. We may suspend an account that breaks these terms, and we'll tell you why where we can.
Governing law
These terms are governed by the laws of the State of Delaware, United States.
