We built the half of the record nobody was keeping.
A chart records what was done to a person. It has no field for how they communicate, what the room does to them, or what to never do without warning. That part usually lives in one person's memory — and gets re-explained, from scratch, at every appointment.
My Care Hub is where that information lives instead. One profile that holds how you communicate, your sensory needs, your medications, your documents and your people — written once, kept current, and handed over in a single scan.
It works for three people at once: the person being cared for, the family caregiver who often holds the details, and the provider who wants to get it right but is meeting someone for the first time.
Why I built this
Luisa Rinaudo

My Care Hub was founded by Luisa Rinaudo, a speech-language pathologist, disability-services provider, and support-needs sibling. After years of watching families repeat the same information to every new provider — and experiencing those challenges firsthand with her own brother — she created the app she wished had always existed.
What we believe
The person's voice stays central
A profile can be managed by a caregiver, but it speaks in the words of the person it describes. Holding the pen is not the same as speaking for someone.
Every field is optional
An empty section is a valid answer. Nobody has to disclose anything to get value from the app.
Sharing is an action, not a state
A profile isn't broadcast. It's handed over deliberately, field by field, and can stop being handed over.
Plain language, always
The app asks how you communicate and what to avoid. It never asks you to have a clinical vocabulary to describe your own life.
Who we are
My Care Hub is designed with input from people with disabilities, family caregivers, speech-language pathologists, direct support professionals, and disability service providers — the people who live this problem, and the people who want to do better than a clipboard.
